Job Title: Palliative Care Social Worker
Employment Type: Full-time
Pay Range: $32.12 - $48.18
Pay is based on experience, skills and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.

Location: Holy Cross Health operates two hospitals and four healthcare centers, all within a short driving distance from Washington, DC, and Baltimore, MD.

Reporting to: Director of the Department
Minimum licensure/certification required (if applicable):State of Maryland LGSW, LCSW or LCSW-C licensure, LCSW-C preferred.
Certification in Palliative Care Social Work preferred.
Minimum Knowledge, Skills, & Abilities Required:Master's Degree in social work required
At least one year of social work experience in a health or mental health care setting. Demonstrated ability to work with all age groups of patients. Able to communicate effectively, demonstrating good audio and visual acuity. Ability to prioritize tasks and manage time effectively. Computer skills in Word, Excel, and ability to learn hospital EMR and other electronic tools.
Role Overview: The Palliative Care Social Worker works in collaboration with the other members of the Palliative Care Program to provide patients and their families with palliative care services including goals of care planning and symptom management. The position supports the Mission of Trinity Health and Holy Cross Hospital
