JOB SUMMARY:

Under limited supervision, this role provides leadership and direction for medication safety and safety improvement strategies. The Medication Safety Coordinator (MSC) is responsible for the coordination of activities related to medication safety as outlined in the UM UCH Patient Safety Program. Activities include but are not limited to reporting, system and process evaluation, formulation of recommendations and initiatives, and education. In addition, the individual will assist with the management of the UM UCH Event Reporting System. The MSC will work collaboratively with all members of the healthcare team. This position requires effective interpersonal and management skills to motivate team members to embrace the UM UCH Culture of Patient Safety. The MSC will lead the culture of zero harm related to all types of medication errors.

Qualifications
Education & Training:

Bachelor's degree in Pharmacy from an accredited college of pharmacy is required; Doctor of Pharmacy (PharmD) from an ACPE accredited college is preferred. Licensed to practice pharmacy in the State of Maryland or eligible for licensure. Applicants whose Maryland licensure is pending by reciprocity from another state will work under direct supervision of a licensed staff pharmacist (for a period not to exceed 6 months) until Maryland licensure is obtained.

Work Orientation & Experience:

Minimum of three (3) years of hospital pharmacy experience is required. Three (3) years of experience in medication or patient safety in a hospital setting is preferred.

Skills & Abilities:

Maintains knowledge of federal, state and local requirements of regulatory and accreditation agencies [The Joint Commission, National Patient Safety Goals, Institute of Safe Medication Practices (ISMP), and CMS] and regulations pertaining primarily to safe medication practices. Knowledge of generally accepted pharmacy principles, practices, and procedures. Ability to use computer systems to manage patient information. Proficiency and demonstrated effectiveness in integrating problem-solving, analytical skills and implementation of targeted or system-wide interventions in the journey to zero harm and incorporating these into new or existing safety programs in coordination with team members, medical staff and hospital leadership. Highly effective oral and written communication skills with the ability to work with team members at all levels of the organization, as well as with regulatory agencies. General knowledge of safety database management with focus on reviewing and investigating safety events reported through reporting sources, including near misses. Knowledge of data measurement, basic statistical analysis, and quality improvement strategies (i.e., root cause analysis, LEAN concepts, etc.). Strong interpersonal skills and a highly collaborative style, inclusive of all stakeholders. Ability to build consensus, provide strong leadership in a team environment. Ability to work with limited supervision in the management of medication/patient safety projects and other safety programs. Strong change management skills with the flexibility and ability to adapt to rapidly changing priorities.
