Job Role Summary
The Infectious Diseases Nurse Coordinator coordinates overall patient care by collaborating with Infectious Diseases physicians, internal departments, external resources, and patients/families to provide high-quality, seamless, and efficient service for patients within their assignment. This position also provides direct care to patients within their assignment. This position functions as a Nurse Care Manager, conducting assessments and implementing care plans for medically complicated patients while demonstrating problem-solving and critical thinking skills, and practicing effective decision-making. Proactively contributes to Eskenazi's mission: Advocate, Care, Teach, and Serve with special emphasis on the vulnerable population of Marion County; models Eskenazi values of Professionalism, Respect, Innovation, Development, and Excellence.
Essential Functions and Responsibilities
1. Serve as coordinator of care, collaborating with physicians, nurses, pharmacists, patients, families, and other hospital departments (i.e. Transition Support, Vascular Access Tean, Imaging, or Primary Care) to provide high-quality care within the area of assignment.
2. Frequent communication with Infectious Diseases to assess patient needs, including social determinants of health and health literacy, and to collaborate with Infectious Diseases to develop an action-oriented and time-specific care plan.
3. Serve as system navigator and primary point of contact for medically complex or otherwise vulnerable patients and families, including those receiving complex outpatient antimicrobial therapy (COpAT) including IV and oral antimicrobials, those requiring repeat imaging, HIV care, or other injectable treatment regimens.
4. Monitor and report on test results for patients in the assignment area. This includes laboratories for monitoring drug levels or safety labs, imaging, or other tests ordered to evaluate the efficacy of the treatment plan.
5. Advocate for the patient/family by collaborating with the multidisciplinary team to implement a care management plan that ensures access and approval of necessary testing, supplies, or other services to aid the patient's recovery. This may include facilitating prior authorization with insurance or assisting the patient in appealing an insurance denial.
6. Performs patient teaching, patient self-management/self-efficacy coaching, and planned care management.
7. Evaluate active funding for each patient and communicate with Financial Counseling to facilitate the initiation of appropriate funding applications.
8. Advises and assists Transition Support with discharge planning for medically complex patients in the area of assignment.
9. Monitor patient progress until the care plan is completed or the patient is released from Infectious Diseases care. This includes ensuring that laboratory tests, imaging, and other orders/referrals are completed as planned and presented to the Infectious Diseases team for review.
10. Develop strong relationships and communication with home health agencies, rehabilitation centers, extended care facilities, long-term acute care facilities, and others involved in the care of medically complex patients.
11. Provide outpatient care of vascular access devices, primarily peripherally inserted central (PICC) lines that include dressing changes, flushing lines, blood draws, and removal of the catheter at the end of treatment. All are to be performed as outlined in Eskenazi Health in the following policies and procedures: Care and Use of Vascular Access Devices, 600-022, and Central Vascular Catheter (CVC) Insertion and Vascular Access Maintenance, 701-3013.
12. Document patient care activities in a clear, correct, concise, coherent, and consistent manner.
13. Participate/Facilitate in outcome analysis and development of discharge pathways that support high-quality patient care.
Job Requirements
• Current Indiana licensure as a Registered Nurse required.
• Two years of clinical nursing experience preferred, one year required.
• Experience in Infectious Diseases is preferred, but not required.
