Senior Community Care of Northern KY PACE is part of Volunteers of America National Services which serves as the Housing and Healthcare affiliate of the Volunteers of America parent organization.

Schedule: Monday-Friday 8:00 AM-5:00 PM (Occasional Weekends required)

Salary: $41.25-$45.00 (Based on years of experience)

About the Job: The Director of Healthcare Services provides direction, coordination, supervision and implementation of professional and supportive services to residents/clients using nursing theory and process in accordance with the Nurse Practice Act. The Director of Healthcare Services assures the residents/clients are treated with respect and dignity, recognizing individual needs and encouraging independence. Perform initial and ongoing assessment of physical & psychosocial needs, coordinate with other departments, agencies and care providers as needed.

Essentials:
  • Provides oversight and direction of the overall operational and clinical effectiveness of the PACE clinic, homecare department, and medication department.
  • Provides oversight, management, and coordination of the consistency and quality of the twenty-four (24) hour care provided to program participants.
  • Provides direct supervision and support of the [CLINIC MANAGER], [HOMECARE MANAGER], [INTEGRATED CARE MANAGER], and the medication department. Provide clinical oversight of nursing and personal care to participants in the PACE Center under the supervision of the [DAY CENTER SUPERVISOR].
  • Oversees the provision of nursing care to ensure all ordered, approved, or care planned nursing care and services are delivered timely and in accordance with nursing standards or practice.
  • In conjunction with the Medical Director and Director of Clinical Support, develop policies and guidelines for participant care (e.g., triage, disease management, adherence to NPA model practices).
  • Develops and oversees the implementation of clinical and other programs to improve participant outcomes and/or improve the quality and effectiveness of care provided, including but not limited to utilization management, residential care coordination, end-of-life/palliative care program, advanced care planning, etc.
  • Responsible for budgetary management of departments and accountable for meeting established financial, operational, and quality targets.
  • Functions as an engaged member of the leadership Coordinate and partner with other leadership team members, assisting in developing and implementing organizational and strategic initiatives.
  • Actively participates and engages in the implementation of quality improvement initiatives. Actively participates as a member of the Quality Assurance Committee.
  • Oversees SCC's infection control and wound care programs to ensure optimal participant outcomes and continuous improvement.
  • Coordinates with contracted service providers (e.g., nursing homes, assisted living, lab, oxygen, ) to ensure effective care and service delivery to program participants.
  • Ensures that direct reports' job responsibilities, authorities, and accountability are defined and understood.
  • Engages in team building, coaches and mentors staff, instructs and teaches staff as needed, and trains clinical staff to function independently.
  • Oversees the system/process for ensuring both compliance and effectiveness of orientation and ongoing competencies for all staff providing clinical services, including registered nurses (RNs), licensed practical nurses (LPNs), certified nursing assistants (CNAs), personal care attendants (PCAs), medication technicians, Ensures that clinical staff work within their scope of practice.
  • Communicate with weekend and after-hour staff, following up with Manages and reviews participants' records, charts, pertinent information, and staff documentation. Documents as needed in medical records according to Senior CommUnity Care gudielines.
  • Educate staff in regard to care and treatment for the elderly as As needed or required, provide nursing care to participants to ensure ongoing continuity of participant care.
  • Establishes and maintains accountability for high customer service standards and monitors, evaluates, and responds to opportunities for improved participant and family/caregiver satisfaction.
  • Maintains the stability and reputation of Senior CommUnity Care by ensuring all activities and operations are performed in compliance with local, State, and Federal laws and regulations and organizational policy.
  • Is responsible for adherence to program/organizational cultural standards, including supporting the development and integration of these standards within the team through modeling, coaching, and accountability.
  • Follows all Senior CommUnity Care policies and procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.
  • Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants and families.
  • Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal precautions.
  • Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.
  • Performs other duties as required

Required Qualifications:
  • Strong ability to create, build and maintain relationships at all levels.
  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
  • Must have a valid driver's license, proof of insurance and have means of transportation.
  • Education: Graduate of a school of professional nursing program required with Bachelors of Science Registered Nursing (BSN). Current State Registered Nurses License and Cardio Pulmonary Resuscitation (CPR) certification required.
  • Experience: A minimum of four (4) years' experience in working with the frail or elderly population required. At least four (4) years of nurse management experience in a hospital, nursing home or community-based setting strongly preferred. Proven experience leading a team of professionals.
