Job Summary:

Responsible for the coordination of the clinical and financial aspects of the care management departments across the south region to ensure appropriate utilization of resources. This includes assessment, planning, and evaluation of the appropriate level of care required along the continuum: outpatient, inpatient, emergency services, home care, sub-acute, hospice, or nursing home care. Collaborates with enterprise utilization, emergency department utilization, and denials management in facilitating appropriate utilization of hospital resources, and facilitates improvement teams and projects to meet hospital outcomes aligned with Froedtert Health strategic goals. Collaborates with enterprise VP Post-Acute Care to develop and implement hospital specific strategies related to readmission reduction, outlier management, length of stay reduction, bundle payments, discharge planning disposition, care transitions and external post-acute partnerships.

EXPERIENCE DESCRIPTION:

Minimum of 5 years of experience in the hospital clinical setting, and 3 years of experience at the upper-middle management level is required. Previous experience in Inpatient/Acute Care Care Management required.

EDUCATION DESCRIPTION:

Master's Degree with healthcare emphasis required. MSN or MBA is preferred.

SPECIAL SKILLS DESCRIPTION:

Interpersonal skills necessary to interact with Medical staff and Hospital management and Administrative staff in a variety of situations. Analytical skills necessary to prepare and interpret statistical reports. Ability to concentrate and pay close attention to detail for up to 75% of work time.

LICENSURE DESCRIPTION:

Current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact) is required OR Licensed in the State of WI as Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) required.
