Community Engagement Navigator

Location: This is a hybrid role and associates may need to report to the office 1-2 days per week. Ideal candidates will be within 50 miles of Woodland Hills, Walnut Creek, Palo Alto, or Costa Mesa, CA.

The Community Engagement Navigator is responsible for for engaging with members to remove barriers to Social Determinants of Health (SDOH) and acting as the communication liaison between the company and referral sources.

How you will make an impact:

  • Engage with members who have been identified as possessing significant SDOH barriers.
  • Work with the member to establish goals and next steps for addressing SDOH.
  • Support member in selecting provider or programs most appropriate for supporting their needs.
  • Interface with HCM to ensure that the individualized plan is aligned with existing or needed case management efforts.
  • Act as a liaison between referral sources and the health plan.

Minimum Requirements:
  • BA/BS degree in Public Health, Social Work, Rehabilitation Counseling or related field and a minimum of 2 years experience with marketing, job development, job placement or social work; or any combination of education and experience which would provide an equivalent background.

Preferred Skills, Capabilities & Experiences:
  • Knowledge of Voc Rehab preferred.
  • Experience with behavioral health or mental health
  • Knowledge of Microsoft Office (Word, PowerPoint, Excel, Outlook, Etc.)

For candidates working in person or remotely in the below location(s), the salary* range for this specific position is $27.57 to $43.14
