Job Summary:
Provides a positive and effective representation of Froedtert Home Infusion and Pharmacy Services and introduces the full scope of services offered to hospital case managers, physicians, nursing agencies and other referral sources. Works closely with the Home Infusion Manager to market and grow Froedtert Home Infusion Services through positive referral source relationships and responsive customer service with referral sources and patients. Collaborates with referral sources to facilitate the coordination of care for new patients and to provide clinical support Coordinates and communicates discharge planning activities relating to home infusion therapy with the Home Infusion Pharmacy, hospital/facility and referral sources. Demonstrates thorough knowledge of core therapy referrals and qualifying criteria by payor source. Ensures ongoing updates and education for referral sources and patients. Assists referral sources and patients with the identification of appropriate candidates for home infusion. Other duties as assigned.
EXPERIENCE DESCRIPTION:
A minimum of three years (3) of experience in a clinical setting as an RN is required. Healthcare Sales/marketing, homecare, infusion or RN Case Management experience is preferred.
EDUCATION DESCRIPTION:
Associate degree in Nursing is required. Bachelors degree from an accredited college or university in marketing, communications, health care or a related field is preferred.
SPECIAL SKILLS DESCRIPTION:
Thorough knowledge of computer systems and the ability to effectively utilize software applications (specifically Microsoft Word®, Excel® and Power Point®) to support Froedtert Home Infusion Services. Good problem solver with the ability to define, clarify and set priorities for producing quality results within established timeframes. Ability to work independently and effectively manage tasks in a fast paced environment. A team player with a positive attitude who works well in a cross-functional team environment. Strong verbal and written communication skills and a sound work ethic. Excellent planning, organizational and time-management skills. Ensures short-term plans are aligned with and supportive of organization goals and long-term objectives. Continually looks for ways to improve performance. Demonstrates a high level of personal motivation and commitment to achieve results. Regularly monitors own performance to ensure goals and expectations are successfully achieved. Establishes and maintains effective relationships with customers.
LICENSURE DESCRIPTION:
Requires current Registered Nurse License in any state or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact).
