Duties: Please Note: This is not a clinical position. Job duties include phone contact with clients and are not sufficient for licensure in PA.

The incumbent will serve as a Care Coordinator for the Steven A. Cohen Military Family Clinic (Cohen Clinic) at the University of Pennsylvania. The Cohen Clinic provides low to no cost, evidence-based behavioral health care to Veterans and military family members. The Care Coordinator will serve as an active member of our care team, supporting established clients during and after their care in the clinic, under the supervision of licensed clinicians. The Care Coordinator will help to coordinator Resource Assessments for incoming clients, support our Case Manager with research and coordination with external referral resources, and conduct regular follows ups with clients to ensure successful referrals. Additionally, the Care Coordinator will be responsible for the completion of Follow Up assessments, with clients who have completed care at regular intervals to ensure maintenance of gains and to assist them in connecting to additional resources as necessary. S/he will maintain timely and complete medical records. The Care Coordinator will also support ongoing clinical data collection and entry efforts, provide cross coverage to our Intake Coordinator and Front Desk representative, as needed, and assist with related research as it arises. Position will include outreach to community partners, including some local travel.

Qualifications: A minimum of a High School Diploma and 3 years to 5 years of experience or an equivalent combination of education and experience required. College coursework or degree a plus.

Related experience (e.g., conducting interviews, case management, care coordination) preferred.
Must pass Pennsylvania s Child Protective Services Act (also known as Act 73) background checks, within 30-90 days of employment. Valid drivers license required.
