The Comprehensive Breast Health Center, located at the 75 Francis Street campus, evaluates women with a variety of breast complaints. As part of the evaluation, patients complete a risk assessment survey. After properly addressing the patient's concerns, those that calculate to have an elevated risk for breast cancer are invited to follow up in the embedded high-risk program known as B-PREP (Breast Cancer Personalized Risk Assessment, Education and Prevention) Program where options for individualized surveillance and prevention measures are reviewed and implemented. Patients are also referred directly in to the B-PREP Program for risk assessment and management. The Breast Program is staffed by a dedicated, multi-disciplinary team of physician assistants, surgeons, and medical oncologists and are supported by an exceptional team consisting of a practice manager, practice assistants, medical assistants, a patient navigator, and research assistants. The team also works closely with colleagues in breast imaging to provide seamless care. The Breast Health RN has an integral role in meeting our mission of providing high-quality, efficient, compassionate care to the patients we serve.
The Staff Nurse will be responsible for:
• Maintaining familiarity with department activity and a close working relationship with clinicians and support staff to ensure that appropriate coordination of care and communication is maintained with clinicians, patients and referring providers.
• Provide clinical telephone triage, especially for urgent clinical situations, alerting appropriate clinicians in a timely manner when indicated
• Review and respond in a timely fashion to all in-basket messages and results for attending physicians, as well as for PA team as directed, eliciting PA/MD guidance as needed. Provide primary in-basket coverage for PA's when out of office
• Preparing and submission of radiology review requests with facilitation of follow up recommendations. Assume primary responsibility for maintenance of the consult log
• Review of pathology consult reports, checking for any discrepancies with outside hospital reports and alerting admin/clinical team when noted as may have implications for care plan
• Complete chart prep and provide in-clinic support for surgeons during clinic sessions. Assume responsibility for following up on any imaging/procedure results and logging cancer developments in spreadsheet shared by clinical team
• Providing pre-operative teaching
• Providing post-operative wound assessment and care with support from PA as needed
• Attending monthly Division staff meetings and other staff meetings as requested
• Participating in departmental and organizational quality management and/or quality improvement projects as directed by the leadership team.
• Proficient use of the electronic medical record, documenting in a manner that is clear, complete, concise, and in SBAR format when appropriate to remain compliant with nursing standards.
• Working in a normal office setting; work entails extended periods of sitting and fast-paced, phone-based care.
• Preparing, performing, and documenting the coordination of clinical care including, but not limited to, calling patients to assess tolerance of recently prescribe chemoprevention, assisting patients with medication questions, delivering lab results, and providing patient education as needed and requested.
• Working collaboratively to provide excellent care to patients and families in the safest environment.
• Other duties as assigned by the supervising physician or leadership team.
