POSITION OVERVIEW:

The Substance Use Disorder (SUD) Program is an article 32 OASAS licensed outpatient program providing culturally affirming, evidence-based counseling and medication assisted treatment to adolescents and adults with SUD diagnoses and their families. Treatment approaches are trauma informed and individualized to the client and family. The program is part of the CCBHC hubs, a Certified Community Behavioral Health Center that serves all members of the community. As part of our person-centered approach, the program is open weekdays and evenings to provide the community with optimal access to care.

The Social Worker is responsible for providing the necessary, culturally competent clinical interventions to individuals of all ages within the realm of the program and incompliance with regulating agencies. Social worker is responsible for providing developmentally appropriate, evidence-based interventions to clients and families, providing crisis intervention, advocating for clients/families, coordinating services with other providers engaged in client's care within and outside the Agency to maximize treatment outcomes.

KEY ESSENTIAL FUNCTIONS (List the functions that are essential to achieve the goals):

    • Conduct assessments to evaluate participant's strengths and resources, current functioning, barriers to engagement, level of risk and identify appropriate interventions to manage risk.
    • In collaboration with the client, family, and treatment team, develop treatment planning to help client achieve goals across multiple life domains, in addition to addressing substance use.
    • Provide evidence-based interventions to enhance motivation to engage in services and support and reduce risky behaviors.

    • Provide individual, group, family, collateral sessions, and crisis services/interventions.

    • Be actively self-reflective & self-aware of how clinical and administrative practice impacts clients, families, and the communities we serve.

    • Approach the work with an anti-racist lens and open to continuously learning and evolving.

    • Liaise with families, schools, criminal justice, and other community agencies to coordinate care.

    • Participate in clinical meetings, staff meetings, trainings, and supervision.

    • Monitor response to treatment interventions using urine toxicology screens, breathalyzers, and other screens to inform the treatment planning process.

    • Open and willing to receive constructive feedback.

    • Maintain documentation according to the standards and time frames established by the Jewish Board, regulatory agencies and/or funding sources.

    • Maintain professional behaviors and ethical standards as established by licensing board, relevant professional association and the Jewish Board policies and procedures.

    • Other duties as assigned.

EDUCATIONAL / TRAINING REQUIRED:

  • New York State License or limited permit in Mental Health Counseling (LMHC) Creative Arts Therapy (LCAT), or social work (LMSW) required along with a New York State Certified Alcohol and Substance Abuse Counseling credential (CASAC).
