Facility: AU Medical Center, Inc.

Responsible for completing initial in-home assessments of patients enrolled into the Wellstar MCG Health Virtual Care @ Home programs. This includes developing the initial care plan, communication with the patient, family, and caregivers around the programs, goals and expectations. The Admissions RN works in coordination with the Manager, Lead Care Coordinator and Virtual Care @ Home Care Team to enroll and consent patients into the program, while coordinating in-person assessments with the patient and family.

Position Requirements:

ADN, BSN, BA/BS with MSN or Diploma in Nursing and 3 years or more of experience in home health, hospice, or in-home nursing experience. Current American Heart Associationor American Red Cross Basic Life Support (BLS), ACLS, current Registered Nursing License (GA) or qualifying multi-state compact license. Current Registered Nursing License (SC) or qualifying multi-state compact license. Valid drivers' license and clean driving history.
